Center for Orthopedic Surgery and Traumatology in Santa Cruz de Tenerife (CECOTEN)

Prosthetic surgery

One of the greatest advances in orthopedic surgery of this century has been the replacement of joints by prosthesis. The joint work of engineers and orthopedic surgeons has enablabed the development of materials and techniques to make this a reality.

Prosthetic surgery is the solution for a damaged and irreparable joint, mainly due to wear and tear of it (osteoarthritis) but also due to traumatic injuries (subcapital fractures in the elderly). It is a surgery with excellent results and consists in replacing a damaged or osteoarthritic joint with an artificial joint called a prosthesis.

 

The knee prosthesis

Because of pain and stiffness, the patient avoids using the joint, so that the surrounding muscles become weak and the mobility of the joint becomes more difficult. When the damage

of the joint is important, the total knee prosthesis allows the patient to resume their daily activities.
The placement of a knee prosthesis is done through a surgical intervention with the patient under anesthesia. The surgeon replaces the damaged joint with artificial materials. In the knee, the bony ends of the femur and the tibia are replaced by prosthetic components that articulate with each other and with the patella (cup), which in most cases is also replaced.

The materials used in total prostheses are designed to allow mobility similar to that of the normal joint. The prosthetic components are usually composed of a metal element surronded in plastic. Various metals are used, stainless steel, cobalt alloys, chromium and titanium. The plastic is polyethylene that is extremely durable and resistant to wear. For the fixation of the components to the bone, cement is usually used, although there are also uncemented prostheses, which are anchored directly to the bone.

 

The prosthesis in the hip

The total hip prosthesis is one of the greatest advances in orthopedic surgery of this century. They are inserted with the aim of replacing a diseased or damaged joint by wear, osteoarthritis, accidents or inflammatory diseases.

The most frequent reason for the placement of a hip prosthesis is to improve the quality of the patient ́s life,due to the pain and limitated movement affecting the development of his daily life.

It is carried out through a surgical operation with the patient under anesthesia. The surgeon replaces the damaged joint with components made of artificial materials. The upper end of the femur is replaced by a metal sphere attached to a shank that is inserted into the bone. A plastic dome is implanted in the spherical cavity of the pelvis that is damaged.

 

The prosthesis in the shoulder

The degenerative problems of the shoulder are one of the main causes of work incapacity. For many people it is a health problem comparable to heart failure or diabetes. The shoulder prosthesis, which could be a solution for many patients, is unknown to a large part of the population.

It is a surgical procedure by which the glenohumeral joint (the joint between the head of the humerus and the scapula) is replaced by a prosthetic implant. This procedure is used mainly in cases of advanced arthritis or severe shoulder arthritis (as in rheumatoid arthritis) to control pain and recover lost functionality; and also after some proximal shoulder fractures in which surgical reconstruction is not possible.
